What Is Secondary Transition?

Secondary Transition is the process for preparing students for life after high school. The transition planning process begins at age 14, or younger if deemed by the IEP team to be appropriate. The transition process is based on the student's interests, preferences, and goals for themselves post-graduation. Transition planning includes six domains; Financial and Legal Supports and Services, Extensive Supports and Developmental Disabilities, Employment, Independent Living and Advocacy, Mental Health and Other Health Related Needs and Community Living and Participation.
